Care team
Our Care team is at the core of delivering compassionate, whole-person care to patients living with kidney disease. This team includes nurse care managers, social workers, dietitians, care coordinators, and more. Each member has a vital role in improving patients’ quality of life. We work together across different specialties, using both medical knowledge and empathy to help with patients’ care.
