How does Evergreen work with my nephrologist?

Your nephrologist leads your care. We’re here to provide extra support during and between office visits. Think of us as an added layer of support to your care team. We help you stay on track with your care plan, answer questions that come up, and share updates with your nephrologist so they’re always in the loop.

Will I have to pay for Evergreen’s support?

There’s no extra cost to work with most of the Evergreen care team. For qualified patients, our support is part of your Medicare health plan benefits. If you see an Evergreen provider for a check-in or consultation, you might have a copay or coinsurance. This works the same way as a visit to any other doctor.

You can check your specialist coverage by calling the member services number on the back of your insurance card.

How do I know if I qualify for Evergreen support?

Evergreen works with certain health plans and kidney doctors (nephrologists) to offer extra support to patients who qualify based on their stage of kidney disease.

We partner with nephrology practices through the Medicare Kidney Care Choices Model (for people with Original Medicare) and with select Medicare Advantage health plans.

 Why am I receiving a call from Evergreen?

Your nephrologist, provider, or health plan has partnered with Evergreen to give you extra support for managing your kidney health. We're reaching out to see how you're doing and to offer help that fits your specific needs. This support is provided at no extra cost to you through your Medicare health plan.

Can my caregiver or spouse be involved in my care with Evergreen?

Yes. Many of our patients have a spouse or caregiver involved in their care, and we encourage it. We’re happy to talk with them, include them in visits, or share helpful information. If it's helpful, we can reach out to your loved one directly. Our team can answer questions or make sure they're included in care decisions.
